What is needed: (Print out the following and bring this with you to your doctors appointment)
Diagnosis codes- This code is needed to fill the claim for your treatment. Your doctor will have the correct code and it will be listed on your prescription.
Frequency of Treatments- The written prescription provided by your primary care doctor must say how often massage therapy treatment should occur (2x per week etc.).
Total Number of Treatments- How many treatments in total should you receive.
Name and UPIN# (Doctor’s ID #) -- Again, your doctor will know this information and will provide that on the prescription.
The written prescription must say how often massage therapy treatment should occur and how long the treatments should last. The physician is free to consult with Kristen about this, as well as to discuss ongoing treatment strategies.
All the paperwork must be filled out prior to receiving your massage therapy treatment, and you must agree to allow Relaxation Works to share information with your physician and insurance company. You also must agree to have the auto / car insurance company pay Relaxation Works directly.
