The process to receiving benefits after an injury

When your insurance company is involved in your claim there is more behind the scenes work that needs to take place.  Typically this process takes up to two weeks to get approval for your claim.   To ensure that you start receiving benefits ASAP you must follow these 3 simple steps first three steps.

Step #1: Download your packet that applies to you

Step#2: Make an appointment with your primary care to get a prescription for massage therapy

You MUST use the prescription form in your packet that you just downloaded.

Step #3 Return two key pages in the packet to my office (snail mail or leave them in my locked mailbox next to office door)

Before I contact your insurance company to get clearance I need to see the prescription from the doctor and Insurance contact information.  I’ve made this process very easy by providing you with a quick  Make sure I get the prescription and “Patient Information to Verify Insurance” form ASAP.  You can bring the remaining pages of the packet with you to your appointment.

That’s it!  I take care of all the phone calls and contact with your insurance company.
